Request for SORRB Classification
Not all cases are eligible to request a risk classification. To determine if a person qualifies, follow the steps below:
Step 1: Download the "Request for SORRB Classification" form using the link below
REQUEST FOR SORRB CLASSIFICATION
Important: Check the eligibility requirements. Some individuals require a court order for SORRB to classify them.
Step 2: Obtain both a Georgia and a national criminal history for the sex offender
Step 3: Obtain all the necessary documents in PDF format (based on the answers to the eligibility questions in the Classification Request form)
For EACH sex offense (including misdemeanor offenses and arrests without conviction*), the following documents are REQUIRED to be submitted in Step 4 (below):
1. Police incident/investigation report (must be the entire fully detailed report, including all supplements)
2. Indictment
3. Disposition/Sentencing Document
For EACH additional offense (including misdemeanor offenses and arrests without conviction*), the following documents are REQUIRED to be submitted in Step 4 (below):
1. Police incident/investigation report (must be the entire fully detailed report, including all supplements)
2. Indictment
3. Disposition/Sentencing Document
* For offenses that did not result in charges/indictment (i.e., just arrest), only the police incident reports need to be summited.
You will need to request and obtain the above documents from the appropriate agencies. For example, incident reports can be requested from the relevant Sheriff's Office or Police Department and indictments and disposition/sentencing documents can be requested from the relevant Clerk of Courts.

Important: All documents (required and optional) must be submitted AT THE SAME TIME as part of Step 4.
Do NOT submit documents on more than one occasion (but due to limits on file size, more than one email may be required). You should obtain all documents BEFORE proceeding to Step 4.
Important: All documents must be in PDF format.
Important: Applications with missing documents will be returned.
Once you have obtained all the necessary documents, go to Step 4 to formally request SORRB classification.
Step 4: Submit the completed Request for SORRB Classification form with all the collateral documents (as an attachment) via email to tracy.alvord@sorrb.ga.gov
